Common spatial patterns analysis for detecting motor imagery in disorders of consciousness

Recent advances in the use of neuroimaging to probe awareness in disorders of consciousness (DoC) have shown that some behaviourally vegetative patients can demonstrate awareness by altering their fMRI activity in response to command. However, fMRI is infeasible for wider use in this challenging patient population. EEG-based methods could provide a robust, portable alternative for testing a much larger number of patients. We focused on Common Spatial Patterns (CSP) analysis for detecting command following in EEG using motor imagery. In healthy controls, CSP analysis of such EEG produces mutually discriminable patterns of volitional motor cortex activity. We tested its efficacy in DoC patients by comparatively evaluating their spatial patterns in a pair of motor imagery tasks against those from controls. Single-trial data from right-hand and toe motor imagery were submitted to CSP analysis to calculate spatial filters that best discriminated the two tasks. We first examined the topographical specificity of the generated filters across controls and patients. Classifiers were applied to the spatial patterns extracted therefrom, and statistically verified accuracies were contrasted with those from direct analysis of electrode readings. Unsurprisingly, spatial patterns observed in patients were more diffuse, and showed increased variability. Nevertheless, in a minority of patients, they produced classification accuracies on par with controls. The results strongly suggested that these patients were able to consistently follow commands by altering brain states at will, providing a striking demonstration of covert residual awareness. This analysis provides strong support for the use of EEG to complement fMRI in broadening the use of neuroimaging in DoC research.
