Relationship between Th17 and regulatory T cells in patients with type 2 diabetes mellitus

Type 2 diabetes mellitus (T2DM) has been associated with immune system disorders through the inflammatory process. Th17 cells promote the inflammatory process as T regulatory cells (Treg) inhibit inflammation. The aim of this study was to determine the relationship between Treg and Th17 in patients with T2DM. We included 19 control subjects and 40 patients with T2DM, 20 patients had less than 10 years of evolution (T2DM<10) and the rest had more or equal to 10 years (T2DM>=10). Th17 and Tregs cells were identified by flow cytometry in peripheral blood mononuclear cells from study subjects. No significant difference was observed in the Th17 percentage of T2DM<10 and T2DM>=10 patients. Regarding Tregs cells, (CD4+, CD25+, FoxP3+ and CD4+ CD25h in theT2DM>=10 patients was observed a statistical difference (p=0.010 p=0.004, respectivetly). When we analyzed all patients together, it was observed a decrease of the three types of cells in patients with T2DM. Furthermore, Th17 cells showed a correlation with cholesterol (r=-0.388; p=0.002); and Treg cells CD4+ CD25h with levels of glucose (r=-0.308; p=0.017) and BMI (r = 0.293; p=0.024). In Conclusion, we found a decrease of Tregs CD4+ CD25+ FoxP3+ and Tregs CD4+ CD25h and Th17 cells, which would indicate chronic systemic inflammatory state present in diabetes, the low number of th17 cells may be due to metformin treatment they were subjected our patients.
