Multidrug-resistant tuberculosis (MDR-TB) presents a problem to global TB control. MDR-TB is refractory to treatment with isoniazid and rifampin the most important drugs for treating TB. MDR treatment involves extended regimens with high rates to failure and deaths. In this study thirteen MDR-TB patients were followed during Directly Observed Therapy (DOT). Blood samples were taken at 0, 3, 6, 12 months and at the end of the therapy. The IL-2, IL-4, IFN-gamma and IL-17 production by specific T cells to ESAT-6, CFP10 and PPD (M. tuberculosis antigens) was evaluated using flow cytometry and TNF-alpha or IFN-gamma using ELISpot assay and the quantitation of 27-soluble molecules in sera using Bio-plex assay. We identify as possible biomarker of therapy successful the decreased of frequency ESAT-6-specific Th17 and Tc17 cells (P≤0.05). Additionally, a decreased of IP-10, IL-8, IFN-gamma, IL-4, IL-9, G-CSF and increased Eotaxin (P≤0.05) were identified as promising biomarkers after 6 months and 1 year of treatment or at the end of MDR-TB therapy. In conclusion, the mycobacterial load reduction after successful therapy it reflected by reduction in the frequency of ESAT-6- specific CD4+ and CD8+ T cell producing IL-17 and serum levels of IP-10, IL-8, IFN-gamma, IL-4, IL-9, G-CSF, and a different behavior of these specific-T cells or molecules would mean a treatment failure.
