CHRONIC LOW BACK AND LEG PAIN, DIZZINESS AND CONSTIPATION RESOLVED WITH FUNCTIONAL NEUROLOGY TREATMENTS IN A PATIENT WITH MULTIPLE MILD TRAUMATIC BRAIN INJURIES

Background: A forty-eight year old female presented to a functional neurology clinic with a history of multiple mild traumatic brain injuries (mTBIs). As a result of her mTBIs, she reported dizziness, tinnitus, insomnia, constipation, light and sound sensitivity, mental fogginess, and chronic low back and leg pain. She reported that these symptoms interfered with her performance of her daily activities and were preventing her from working. Methods: Patient exam revealed mild paresis was noted on the left side of her face. Moderate palatal paresis was evidenced on the left and worsened with repetition. Examination of the lower extremity exhibited a temperature differential (colder on the right) and diminished pedal pulse on the right. Decreased sensation was noted over L4, L5 and S1 dermatomes on the right. Alternating palms up palms down testing revealed dysdiadokinesia on the right. Finger tapping test was inaccurate on the right and slow on the left. Point localization showed less accuracy with the right arm and leg. Heel to shin testing was slower and less accurate on the right. Gait examination revealed a bilateral decrease in arm swing. Dual tasking further decreased arm swing bilaterally. The patient underwent treatment for five consecutive days, three sessions daily. Each session consisted of 3 sets of complex passive proximal joint stimulations, left brain eye exercises, 1-2 sets of breathing and palatal exercises and specific whole body off-vertical-axis rotations. In addition, specific gaze stabilization exercises, trigeminal and hypoglossal nuclei stimulation, optokinetic stimulation, and clapping in time to a metronome were also utilized. Results: Patient experienced complete resolution of dizziness and unstable gait. Additionally she reported that her pain and constipation were resolved. Normal facial tone and normal sensation of her right leg had returned. She was no longer sensitive to light or sound. She experienced increased mental clarity and focus. Her motor tests had also returned to normal. Conclusion: The rapid and substantial improvement that was evidenced in this case suggests further research into functional neurology treatments for patients dealing with symptoms from mTBIs.
