CLINICAL NUTRITION PROGRAM IMPROVES MENIERE’S SYMPTOMS IN 62 YEAR OLD MALE WITH TYPE 2 DIABETES

Background: A 62 year old male presented with chief complaints of vertigo and hearing loss. Onset of symptoms was 4 years prior to presentation and began with sudden rotatory vertigo and nausea with subsequent hearing loss, now worse in the left ear. Upon presentation he was experiencing two to three episodes of rotatory vertigo per week. Medications included metformin, gabapentin, bisoprolol and hydrochlorothiazide, diltiazem, canagliflozin, and meclizine as needed. He noted numbness and coldness in the feet that disturbed his quantity and quality of sleep. His vertigo was characterized by feeling his body spinning to the right and the world is moving left. Quick movements of the head, primarily to the left, exacerbated his symptoms but sometimes vertical movements of the head could elicit the symptoms. Fatigue, hunger and stress all exacerbated his chief complaints. Methods: Neurological examination revealed loss of vibration sense on bilateral first toes (Rydell-Seiffer grade 3.5), a positive left Halmagyi’s head thrust for both the left horizontal canal and the left posterior canal, excessive Romberg’s position showed an excessive speed of sway in the AP plane although he did not lose his station. There was dystaxia, dysmetria or dsydiadochokinesia in the arms or legs. VNG testing revealed no abnormalities. Blood chemistry revealed elevated fasting glucose (135), elevated HbA1c (6.8), ferritin (840), elevated Cardiac C-reactive protein (6.6) and vitamin D deficiency (18.3). Patient was placed on an anti-inflammatory diet and supplement protocol featuring vitamin D, turmeric, resveratrol, Omega fatty acids, methylated B vitamins, glutamine and multivitamin/mineral with therapeutic levels of chromium and vanadium. Results: After thirty days of treatment, patient reported he had suffered no episodes of vertigo since beginning treatment. He reported weight loss of fifteen pounds. He was able to sleep all night without waking due to the paresthesias in his feet. Halmagyi’s head thrust was negative for left horizontal and posterior canals. After 90 days of treatment, the patient reported only one brief fifteen episode of mild vertigo that was likely triggered by consumption of foods not allowed on his anti-inflammatory diet. Retest of at 90 days showed a reduction of HbA1c to 6.1. Conclusion: The author suggests that further investigation into clinical nutrition and supplementation in the treatment of Meniere’s disease symptoms, especially in patients with objective hyperglycemia.
