SUPPLEMENTATION WITH METHYLFOLATE AND FREE-FORM AMINO ACIDS IMPROVES THIRTY-FOUR YEAR HISTORY OF INSOMNIA IN A MAN WITH NORRIE DISEASE

Background: A 34 year old male with Norrie Disease was presented by his mother for chief concerns of inability to sleep through the night since he was born, seizures refractory to medication, unpredictable behavior, outbursts of anger and obsessive thoughts. At time of presentation he was taking Zonegran®, Lamictal XR®, Topamax®, Banzel®, Clonazepam®, Risperdal® and a variety of vitamins and minerals. Methods: Blood chemistry revealed elevated creatinine (1.29 mg/dL), GGT (71 IU/L) as well as clinically significantly low cholesterol (142 mg/dL) and folate (12.4 ng/mL). Urinary organic acids testing revealed non-detectable levels of citrate, low levels of cis-aconitate, homovanillate, and pyroglutamate, as well as clinically significantly low levels of isocitrate and 5-hydroxyindoleacetate. In addition, he had high levels of 2-methylhippurate and clinically significantly high levels of indican. The patient was allowed to continue his current regiment of vitamins, minerals, omega fatty acids and probiotics. He was started on a blend of free-form amino acids (l-histidine, l-leucine, l-isoleucine, l-lysine, l-methionine, l-phenylalanine, l-threonine, l-valine, l-alanine, l-arginine, l-glutamine, glycine, l-serine, l-tyrosine, l-tryptophan) and 1,000 mcg/day of l-methylfolate. Results: After 30 days of treatment, the mother reported that, since beginning treatment, the patient had slept through the night uninterrupted every night with the exception of two nights. She also reported significant improvement in his unpredictable behaviors and anger outbursts. He maintained these improvements over the next nine months. Retesting of his urinary organic acids showed improved levels of all the previously abnormal levels. When the mother introduced a diamin oxidase enzyme supplement, his sleep worsened. After removing that supplement, his sleep normalized. A second retest after 14 months of treatment showed that all the urinary organic acid levels had normalized. At last follow up--18 months after starting treatment--the patient continued to sleep six to eight hours uninterrupted through the night. Conclusion: The author suggests further investigation into using supplementation--specifically methylfolate and free-form amino acids--in the treatment of associated symptoms of Norrie Disease.
