OCULAR SKEW DEVIATION, OBSESSIVE COMPULSION, DEPRESSION AND ANXIETY IMPROVED THROUGH MULTIMODAL NEUROREHABILITATION

Background: A 50-year-old, right handed, Caucasian, female was referred to our clinic with past history for head trauma and significant OCD thought patterns. She reported having anxiousness, depression and OCD starting from young age (age 6). She had significant brain trauma and broken ribs, falling off a horse (age 17), as well as, after an auto accident (age 48). She was hospitalized for these same cognitive patterns at ages 20 and 47. Collaboration with her referring counselor was made to aid in brain spotting. The patients goal was to reduce her symptoms of OCD, depression and anxiety
Methods: A thorough functional neurological physical exam was performed with frequent bedside vestibular and ocular motor examinations/monitoring. Computerized dynamic posturography, Maddox Rod testing, Unified Parkinson's Disease Rating Sale for finger tapping, alternating hand movements was used to monitoring progress. Multimodal vestibular ocular therapy, multiplane joint movement and manipulation was performed. Collaboration with her counselor who performed Brain Spotting was performed in-clinic to help with cognitive brain therapy outside the clinic and enhance treatments.
Results: Over a seven-month time period of 12 visits and brain-based home exercises her subjective self-reported feelings of anxiety, depression and OCD symptoms reduced to 0/10 from 8-9/10. Minor recurrence of symptoms occurred, and home-based brain exercises were updated which helped reduce symptoms again.
Conclusion: Multimodal neurorehabilitation patient centered focused on vestibular therapy and ocular alignment/motor programs is important in neuropsychological cases. Neurorehabilitation should include home-based brain exercises and collaborative care. The authors suggest further research utilizing multimodal collaborative care to improve neuropsychological outcomes
