Abstract: Functional neurology treatment of a 15 year-old female with concussion symptoms including prosopagnosia.

Background: A 15 year-old female presented to our clinic for concussion symptoms that she sustained 6 weeks prior. Her initial complaints were headaches, memory loss (anterograde and retrograde), prosopagnosia, photophopia, phonophobia, sleep disturbance, dizziness, balance problems, poor coordination, difficulty reading, poor comprehension and poor vocabulary. She was unable to participate in school.
Objective: Decreased delayed memory and poor sequencing. Decreased graphesthesia, stereognosis and double simultaneous stimulation on the left. Poor balance in Romberg’s stance and she swayed to the left in Tandem stance. Ataxia was noted on left finger-to-nose and left heel-to-shin. Grade 2 UPDRS Finger-tapping on the left and Grade 1 Finger tapping on the right. She had a short stride length on the left with left lateropuslsion; gait worsened with dual tasking. Decreased ability to mirror hand images on the left when in the left field of vision. Convergence insufficiency was noted. Videonystagmography testing notable for saccadic intrusions on horizontal and vertical pursuits, hypometric saccades and increased blink rate on left gaze and downward gaze.
Intervention and Outcomes: A customized treatment plan consisting of vestibular therapy and brain based rehabilitation was performed at a frequency of three times per week for two months and then decreased to once per week for two months. At the resolution of care she had normal gait, balance, coordination and ocular motor function. Long term memory and the ability to recognize faces returned. She was able to return to regular classes.
Conclusion: This author recommends further investigation into the treatment of concussion and prosopagnosia with vestibular rehabilitation, ocular motor therapy and brain based rehabilitation.
