Developing a Prototype Naming Therapy Application in Malay for People with Aphasia

Introduction
Word-finding difficulty or anomia is a common symptom amongst people with aphasia. They failed to find the suitable words while communicating, causing an interruption in the flow of verbal interaction that can discourage the desire to communicate. Over the years, treatment of anomia mainly used objects and static pictures to help improve word-finding skills. However, recent technological advancements have had a drastic impact on the way people carry out naming therapy especially using applications in mobile devices.
A number of mobile device applications for naming therapy have been developed in different languages i.e. English, French, German and Spanish, but no application has been developed yet for the Malay speaking individuals with aphasia. Based on the theoretical principals of Semantic Feature Analysis (SFA) and Phonological Components Analysis (PCA), naming therapy application using the built-in cueing therapy in Malay is developed to help in improving the retrieval of words.
Objective
The study aims to develop a beta version prototype of naming therapy application in Malay that can be enhanced in mobile devices for people with aphasia who suffers from word finding difficulty using both semantic and phonological tasks.
Methods
A digital flashcard application prototype consists of customised user interface (UI) and a data collection module was developed. Ten Malay nouns, matched and controlled for imageability, familiarity and word length were selected and transformed into coloured photographs within this prototype. The prototype was divided into three main UI: 1) Learning page; 2) Testing page; and 3) Result of assessment. The procedure for each UI is as follows:
i) Learning page, patient will be asked to say each word. If he/she is unable to say the word spontaneously, cues will be given to him/her by pressing the button. The green button will be pressed as a checkmark as soon as the person gets the correct answer.
ii) Testing page, the patient will be asked to say each word without any clues. The green button will be pressed as a checkmark as soon as the person gets the correct answer.
iii) Result of assessment page is used for speech-language therapist (SLT) or family members to share and monitor his/her progress.
During the treatment, cues are also provided. There are five steps of cueing, organised in a hierarchy from the least to most helpful (from definition on the top to repetition on the bottom). The cues given are a mix of semantic (definition of function), orthographic (first letter and printed word), and phonemic (first syllable and word repetition). The cues in this prototype are accessible to be used in any order.
Conclusion
This prototype is now ready to be used and a usability testing which covers mental model, metaphor, visibility, affordability and feedback are proposed in evaluating the application effectiveness. The results of this study will be essential in developing the application for comprehensive aphasia therapy in the future.
