The brain responses of the difference between cognitive processing and the emotional discrimination of emotional stimuli in schizophrenia

The aim of this study was to investigate the brain responses to the difference between cognitive process and emotional discrimination in the participants with schizophrenia. The participants were sixteen right-handed healthy volunteers and sixteen right-handed patients with schizophrenia. The stimuli were composed of emotional pictures and words (words represented main theme, words related to theme, words unrelated to theme) with positive emotional content, negative emotional content and null trials that were presented in an event-related design. In the emotional discrimination condition, they had to discriminate positive, negative or neutral emotion of the presented emotional pictures during fMRI. Then, all participants had to find main theme of the same emotional pictures (finding main theme condition). A spin echo, T2-weighted sequence with 30 contiguous axial slices was acquired on a 3T MR scanner for the acquisition of a T2*-weighted gradient echo planar imaging (EPI) sequence sensitive to the BOLD contrast [TR = 3000 ms, TE = 35 ms, flip angle 90°, with matrix of 64x64 and field of view of 220 mm]. Event-related data for fMRI were analyzed by SPM (Statistical Parametric Mapping) 2. In the comparison of the main theme condition with emotional discrimination condition, normal participants showed brain activations in the bilateral inferior parietal gyrus, the superior temporal gyrus and the bilateral orbitofrontal gyrus (uncorrected p<0.005, cluster size>30). On the contrary, the patients with schizophrenia showed brain activations in the precuneus and the inferior occipital gyrus. In the emotional discrimination condition, normal participants showed brain activation in the middle temporal gyrus, the inferior temporal gyrus, the fusiform gyrus, the bilateral hippocampi, the superiror parietal gyrus, the precentral gyrus and the middle occipital gyrus. In the finding main theme session, the participants showed brain activations in the precentral and postcentral gyri as the positive trials were compared with negative trials. On the contrary, the patients with schizophrenia showed brain activations in the precuneus and the middle frontal gyrus. In negative trials, normal participants showed brain activations in the middle occipital gyrus and the cerebellum. However, the patients with schizophrenia showed brain activation in the lingual gyrus and the cuneus. In this study, we could demonstrate the different cerebral activations between the normal participants and the patients with schizophrenia depending on the task. In the finding main theme condition, the normal participants showed brain activations in the regions related to decision making including the orbitofrontal gyrus. However, the patients with schizophrenia showed brain activations in the regions related to emotional processing including the precuneus.
