Periodontal disease and its relationship with ultrasensitive C-Reactive Protein (hsC-RP): a prospective, randomized, cross-sectional, case-control study in subjects with type 2 diabetes.

Aim. The aim of this research is to study the periodontal status of patients with type 2 diabetes mellitus, verifying its impact on systemic inflammation. Materials and Methods. A total of 93 individuals were studied, 47 patients with type 2 diabetes mellitus and 46 non-diabetic individuals of similar age and sex. The periodontal status was analyzed with the following indexes: oral hygiene index, calculus index, bleeding on probing index, periodontal pocket depth and attachment loss. Venous blood samples were obtained to analyze the systemic inflammation from the serum levels of inflammatory mediators such as ultrasensitive C-Reactive Protein (hsCRP), TNF-α and IL-6. Results. There were no differences between the diabetic group and the control group with regard to plaque index, calculus and bleeding on probing. The mean depth of the periodontal pockets of the diabetic group was 2.53 ± 0.96 mm and 2.11 ± 0.59 mm in the control group, the depth of the periodontal pockets being higher in the diabetic group (p = 0.014). Similarly, the average attachment loss was also higher in the diabetic group (p = 0.028). A greater value was observed in both the index of bleeding on probing and the depth of the periodontal pockets in the poorly controlled (PC) group with respect to the control group. The hsCRP levels were significantly higher in the group of patients with type 2 diabetes mellitus (p = 0.005). A correlation between hsCRP levels and periodontal status in diabetic patients was observed. The diabetic patients presented worse periodontal status than the controls, with a greater average depth of periodontal pockets and attachment loss in respect to the control group. The average depth of the periodontal pockets in diabetic patients with a poor metabolic control was greater respect to control group. Discussion. In conclusion, diabetic patients presented a worse periodontal status than control patients, with a greater average depth of periodontal pockets and attachment loss, worsening the average depth of periodontal pockets when the metabolic control worsens. There is a tendency to increase the systemic levels of hsCRP due to an increase in the calculus and bleeding on probing indexes, indicating a tendency to increase inflammation when the periodontal status worsens.
