Leveraging Web of Science (WoS) and VOSviewer for data collection in-depth bibliometric analysis respectively, we meticulously done a bibliographic analysis encompassed bibliographic coupling, co-citation, coauthorship, and co-occurrence trends. The results unveiled a total of 1045 publications, highlighting a steady rise in global research interest and publication rates. China dominated in terms of publication count, H-index, prolific authors, and funding, while the United States of America (USA) showcased its research prowess with the highest total citations, as well as strong average citation and H-index rankings. The journal Pharmaceutics led in publication volume, followed by the Journal of Controlled Release with the most robust total link strength. Leading institutions like Shanghai Jiao Tong University, Zhejiang University, and Harvard University were at the vanguard of this research.VOSviewer analysis identified four primary research clusters, which are tissue engineering, cancer, neurological diseases, and targeted delivery, while Citespace clustering further distilled this into ten key thematic areas, including differentiation, tissue regeneration, drug resistance and etc. This pioneering visualization and bibliometric assessment not only offer a holistic overview of the developmental trends and intellectual structure in the field of MSC-EVs for drug delivery over the last decade but also accentuates the increasing global importance of this field, particularly highlighting the pivotal roles of China and the USA. These insights lay a foundation for guiding future research trajectories and spotlighting the nascent frontiers in MSC-EV-based drug delivery..
