Typhoid fever with fatal intestinal hemorrhage is rare in the antibiotic era. Acute acalculous cholecystitis (AAC )is also particularly rare in adult typhoid fever cases. Here, we described a rare adult case of typhoid fever with massive lower gastrointestinal bleeding and AAC due to Vi-Negative and antimicrobial resistant Salmonella enterica subspecies enterica serovar Typhi (S. Typhi) infection. To our knowledge, this is the first adult case of two complications due to Vi-Negative and antimicrobial resistant S. Typhi infection. Our aim is to alert physicians to pay attention to this rare condition.
