Structural inequities represent a persistent and profound challenge to the health and well-being of LGBTI+ populations around the world. Rooted in longstanding historical, legal, and cultural systems, these inequities are perpetuated through discrimination, marginalization, and exclusion from health care and public policy. Despite increasing visibility, LGBTI+ individuals frequently confront systemic barriers, such as criminalization, lack of legal protections, inadequate representation in decision-making, and limited access to culturally competent care. These issues are further complicated by intersecting forms of oppression across ethnicity, socioeconomic status, gender, and geography.

This Research Topic aims to advance understanding of how power, policy, and social institutions shape health outcomes for sexual and gender minorities. By examining structural drivers of inequality and highlighting resistance and participatory models, we seek to promote transformative approaches to inclusive health systems and policy-making. The ultimate goal is to inform bold, evidence-based strategies for advancing health equity and social justice in LGBTI+ communities worldwide.

To gain further insights into these complex challenges, we welcome articles addressing, but not limited to, the following themes:

Structural stigma, discrimination, and marginalization

Intersectionality, compounded disadvantage, and privilege

Health equity and human rights-based approaches to healthcare

Policy barriers and enablers in health systems

Community advocacy, participation, education, and empowerment

Cross-national or cross-cultural policy comparisons

Legal and institutional frameworks impacting access to care

We invite innovative, high-quality submissions from all disciplines—including original research, policy reviews, theoretical essays, case studies, and community reports. Both qualitative and quantitative research, as well as mixed-methods and theoretical work, are encouraged. We particularly welcome contributions from networks and alliances supporting inclusive LGBTI+ health policies and practices.

This Research Topic is paired with “Health Inequalities in LGBTI+ Populations: Biopsychosocial Perspectives Across the Life Course,” which explores the biological, psychological, and social dimensions of health disparities in LGBTI+ communities.
