Background

Endometriosis is a leading cause of chronic pelvic pain and infertility, yet diagnosis is frequently delayed, with an average time from symptom onset to diagnosis of approximately 6–9 years. This delay is particularly concerning for adolescents and young people, in whom symptoms often begin early but are frequently normalised, under-recognised, or attributed to primary dysmenorrhoea. As a result, opportunities for early intervention and disease modification are often missed.

This Research Topic aims to bring together current and emerging research focused on non-invasive and minimally invasive diagnostic strategies that may help reduce diagnostic delay, especially in younger populations.

We welcome original research articles, systematic reviews, and clinical perspectives addressing areas such as:
- Advances in imaging, including expert and advanced ultrasound techniques, MRI, and emerging imaging modalities;
- Biomarker research, including circulating, molecular, inflammatory, and multi-omic approaches
- The application of artificial intelligence and machine learning to improve diagnostic accuracy and prediction;
- Development and validation of clinical algorithms and risk stratification tools, including those applicable to adolescents;
- Patient-centred diagnostic pathways and their impact on time to diagnosis and long-term outcomes;

Overall, the aim of this Research Topic is to highlight translational and clinically meaningful work that may help shorten time to diagnosis, facilitate earlier recognition (particularly in adolescents) and ultimately improve outcomes for people living with endometriosis.
