Venous thromboembolism (VTE), comprising deep vein thrombosis and pulmonary embolism, remains a significant source of morbidity and mortality despite considerable advances in imaging modalities and therapeutic approaches. While clinical assessment tools for VTE have evolved, substantial limitations persist in accurately stratifying patient risk for recurrence, bleeding complications, and long-term sequelae. Traditionally, risk models are based on demographic factors, comorbidities, and a narrow spectrum of laboratory measures, primarily D-dimer, which often fall short in specificity and prognostic value across varied clinical contexts. Recent research underscores the heterogeneity of VTE, recognizing it as a multifaceted disorder at the intersection of coagulation, immune activity, inflammation, endothelial function, and genetic susceptibility. The urgent need for more refined and biologically informed approaches to diagnosis, prognosis, and tailored management has prompted a growing interest in the identification and validation of novel biomarkers that reflect these underlying pathways.
This Research Topic aims to bridge mechanistic insights with real-world clinical application by advancing the study of emerging and novel biomarkers in VTE. A central objective is to foster research that not only elucidates the mechanistic underpinnings and clinical utility of biomarkers, from circulating molecules to molecular signatures, but also demonstrates their impact on patient assessment and management in diverse populations and care settings. By emphasizing rigorous validation, multimodal integration of biomarkers with existing clinical tools, and frameworks for practical implementation, this initiative aspires to accelerate the translation of biological discoveries into meaningful advances for precision medicine in VTE. Key questions include which biomarkers improve diagnostic precision, predict recurrence or bleeding, and enable more individualized therapeutic strategies, as well as how best to standardize and operationalize biomarker-based approaches in routine care.
The scope of this Research Topic encompasses integrative studies of emerging VTE biomarkers for risk stratification, prognosis, and personalized clinical decision-making, while acknowledging potential limitations related to reproducibility, standardization, and diverse patient cohorts. Contributions are encouraged across basic, translational, and clinical research addressing relevant topics. To gather further insights in the intersection of biomarker innovation and venous thromboembolism management, we welcome articles addressing, but not limited to, the following themes:
• Evaluation of circulating biomarkers for diagnosis, risk stratification, and assessment of disease severity
• Identification and validation of biomarkers linked to recurrence, bleeding risk, and long-term complications such as post-thrombotic syndrome and chronic thromboembolic pulmonary hypertension
• Integration of biomarker data with clinical scoring systems, imaging findings, and therapeutic decisions for individualized management
• Development and assessment of biomarker-based strategies to guide anticoagulant selection, treatment duration, and monitoring
• Performance and applicability of biomarkers in diverse and real-world patient populations
• Standardization, reproducibility, and clinical implementation of novel and established biomarkers
• Creation of new diagnostic and prognostic tools for translation into clinical practice
