Background

Precision medicine is rapidly expanding beyond human genomics to incorporate the microbiome: the complex microbial ecosystem that shapes human health, metabolism, and immunity. Advances in multi-omics, including metagenomics, transcriptomics, metabolomics, and lipidomics, have generated unprecedented insight into how host–microbiome interactions drive disease susceptibility and individual treatment responses.

Yet translating this high-resolution multi-omic knowledge into actionable clinical decisions remains a significant challenge. Bridging systemic microbial profiling with individualized interventions requires robust integrative frameworks. As the volume and complexity of patient-level data grow, coherent approaches to data integration, synthesis, and clinical application across physiological axes (such as the gut–liver and gut–brain axes) are essential.

This Research Topic aims to strengthen the connection between molecular microbial research and individualized patient care. It highlights innovations that exploit host–microbiome profiling to maximise therapeutic benefit and minimise harm. By bringing together research across diagnostics, multi-omics, and immunology, this collection will contribute to a more unified and clinically actionable microbiome-driven precision medicine framework.

We welcome articles addressing, but not limited to, the following themes:

• Multi-omics (metagenomics, metabolomics, lipidomics) in host–microbiome profiling
• Microbiome signatures as predictive and prognostic diagnostic biomarkers
• Individualized interventions targeting the gut–systemic axes (e.g. gut–liver, gut–brain)
• Host–microbiome interactions in modulating immune responses and therapeutic efficacy
• Methods for complex patient data collection, integration, and synthesis
• Translational pathways from microecological discovery to individualized clinical care
