Axial spondyloarthritis (axSpA) is a chronic inflammatory disease that affects the axial skeleton and comprises radiographic axSpA which has visible sacroilitis and nonradiographic axSpA which has no diagnostic radiographic sacroiliitis. Structural damage is the main contributing factor for patient status in longstanding disease. Syndesmophytes and sacroiliac joint ankylosis are the main features of structural damage in axSpA. It has been postulated that both cytokines and non-immune pathways such as the Wingless (Wnt)/β-catenin signaling and bone morphogenetic may play crucial role in radiographic progression of axSpA.
Important advances have been made in the understanding of the pathogenesis of radiographic progression in axSpA. But, the data about the mechanism and effective management of radiographic progression in axSpA is still limited. Better understanding of the pathogenesis of AxSpA will help to effective management of both clinical and radiological aspects of disease
