Stroke incidence and prevalence are increasing, causing substantial socioeconomic burdens. Timely acute treatment, such as thrombolysis, are important to reduce mortality and disability, but cannot prevent many stroke survivors having to live with disabilities after acute care.
Many stroke survivors and their caregivers often lack a personalized long-term continuous care plan and important information at time of discharge from acute treatment. Because of the long-term care needs being unmet in many cases, subjective and objective outcomes remain unfavorable.
Therefore, adequate transitional and long-term care strategies after stroke are needed, and their implementation easy to adapt to the different health care systems throughout the world, and changing environments that we have experienced with the recent COVID-19 pandemic.
The goals of this Research Topic are as follows:
1. To find out the different needs for transitional and long-term care among stroke survivors in different health care systems.
2. To find evidence for various interventions in transitional or long-term care after stroke.
3. To find out barriers or facilitating factors to implement transitional or long-term care after stroke.
4. To know about the different situations and challenges across different countries regarding transitional or long-term care after stroke.
5. To investigate the novel approaches for more effective transitional or long-term care after stroke.
We welcome research involving but not limited to:
• Epidemiological studies for disabilities or unmet needs for long-term care service, including rehabilitation in stroke survivors such as cross-sectional or cohort studies with enough sample size.
• Clinical trials including meta-analysis, a randomized controlled trial, pragmatic trial, controlled study to prove the efficacy of interventions during transitional or long-term care, or to investigate the barriers or facilitators.
• A minireview on specific transitional or long-term care systems across countries and the challenges they face, but the number of minireviews to be considered in this topic will be limited.
• Studies with the application of novel approaches to solve the problems in the current post-stroke transitional or long-term care system. Novel approaches can include technologies such as ICT or wearable devices, but they should focus on improving transitional care compliance or outcome.
