In this edition of Frontiers in Pediatrics, authors will discuss a wide-variety of pediatric diseases which often lead to severe Illnesses and hospitalizations for children in low-resource settings including certain populations in high-income countries such as Native Americans/Alaskan Natives, refugees, and immigrants. We will focus on the current management in those settings including recent updates, looking at adaptations of care when appropriate. We will attempt to discuss the most common diseases seen in the majority world where at least 80-90% of pediatric deaths occur.
Topics to be addressed will include infectious diseases such as malaria, HIV, tuberculosis, rabies, COVID and other vaccine preventable diseases like diarrhea, measles, tetanus, poliomyelitis, pneumonia. Nutritional and gastrointestinal conditions such as severe acute malnutrition and micronutrient deficiency diseases like Beri-Beri and zinc deficiency will be addressed. Fluid and electrolyte management will also be discussed, as will conditions associated with vomiting, diarrhea, dehydration and fluid management in sepsis and other pediatric shock disorders. Other conditions to be considered include renal diseases; neonatal problems such as prematurity, severe neonatal hyperbilirubinemia, hypoxic ischemic encephalopathy and hematological disorders including sickle cell disease, glucose-6-phosphate dehydrogenase deficiency, iron deficiency anemia, and lead exposure. Other topics include: simulations in low-resource settings; respiratory and cardiovascular problems including asthma, pneumonia, acquired and congenital heart diseases; neurological disorders such as seizures, auto-immune disorders, and cerebral palsy. We will also discuss drug abuse, child abuse, trauma, burns, poisonings and other pediatric emergencies. Finally ethical issues including determination of appropriate treatments, and the right approach for practitioners from high income countries practicing in low-resource settings such as practicing medicine as a partnership and determining how to do so ethically. While updates in management will be highlighted, these are constantly changing, and readers will be advised to always verify the most current guidelines for each disease from other appropriate sources including in-country guidelines when available.
