Pediatric anesthesia presents unique challenges and considerations due to the anatomical, physiological, and developmental differences between children and adults. The perioperative management of children requires a nuanced approach to ensure safety and efficacy while minimizing potential long-term adverse effects. Recent research has highlighted the impact of anesthetic agents on neurodevelopment, raising concerns about the potential for long-term cognitive and behavioral deficits. Additionally, the management of airway and ventilation in children is critical due to their smaller airway size and increased risk of respiratory complications. Effective perioperative volume management and strategies for blood transfusion are essential in preventing hemodynamic instability and ensuring adequate tissue perfusion. The use of peripheral nerve blocks for postoperative analgesia is increasingly recognized as a valuable tool in pediatric anesthesia, offering effective pain control with reduced opioid requirements. Advancements in perioperative monitoring, including the use of ultrasound, have the potential to improve safety and outcomes in children.

The goal of this Research Topic is to explore and evaluate innovative perioperative management strategies in pediatric anesthesia that aim to improve patient outcomes. We seek to address the challenges associated with anesthetic techniques, airway management, and perioperative care, with a focus on optimizing neurodevelopmental and behavioral outcomes. By gathering contributions from leading experts in the field, this Research Topic aims to enhance understanding and promote evidence-based practices that ensure the safety and well-being of children undergoing anesthesia. In addition to clinical and technical considerations, broader themes such as perioperative outcomes and quality improvement, pharmacology and access to safe analgesia, and health-systems, policy, equity, and ethics are equally important in shaping the future of pediatric anesthesia care, particularly in diverse global contexts.

Authors are invited to contribute original research, reviews, and case studies on the following themes:

• The Role of Type and Depth of Anesthesia on Neurodevelopmental and Behavioral Deficits: Investigate how different anesthetic agents and their depths impact pediatric neurodevelopment and behavior post-surgery.
• Airway and Ventilation Management: Explore innovative techniques and strategies to optimize airway management and ventilation in children.
• Perioperative Volume Management and Blood Transfusion: Evaluate strategies for managing perioperative fluid balance and blood transfusion in children, focusing on improving safety and outcomes.
• Peripheral Nerve Block and Postoperative Analgesia: Examine the efficacy and safety of peripheral nerve blocks in managing postoperative pain and reducing opioid use in children.
• Perioperative Monitoring and Ultrasound: Assess the role of advanced monitoring techniques and the use of ultrasound in improving perioperative care and outcomes in pediatric anesthesia.
• Perioperative Outcomes & Quality Improvement: Focus on the epidemiology of perioperative morbidity and mortality, development of clinical databases, and implementation of quality improvement initiatives, particularly in resource-limited and rapidly developing settings.
• Pharmacology & Analgesia: Exploration of access, affordability, and efficacy of anesthetic and analgesic agents; opioid-sparing strategies; and advances in regional anesthesia for pediatric patients.
• Health-systems & Policy, Equity & Ethics: Examination of financing models, supply-chain challenges, and the role of national surgical plans in strengthening pediatric anesthesia services. This theme also addresses social determinants of health, gender considerations, and strategies to ensure equitable representation of underserved populations in research and clinical practice.
