Glioma, originating from glial cells, is the most prevalent primary malignant brain tumor in adults. Glioma accounts for approximately 80% of all malignant brain tumors, with an incidence of about 5 to 6 cases per 100,000 individuals globally. According to the latest WHO Classification of Tumors of the Central Nervous System (the fifth edition), gliomas are categorized into WHO grades 1 to 4. Among these, grade 4 glioblastoma exhibits the poorest prognosis, with a median survival time of only around 14.4 months.

The current standard treatment for glioma involves maximal safe resection, followed by concurrent chemoradiotherapy and sequential adjuvant chemotherapy. Chemotherapeutic agents such as temozolomide, procarbazine, lomustine, and vincristine are commonly employed in glioma management. Notably, temozolomide is a key chemotherapeutic agent for glioma due to its oral administration and relatively mild adverse effects. However, with the occurrence of clonal evolution, glioma cells progressively acquire drug resistance mutations, leading to chemotherapy resistance, which poses significant challenges to glioma treatment. Therefore, elucidating the mechanisms underlying chemotherapy resistance in glioma and implementing targeted interventions will optimize precision medicine approaches for this specific patient population, ultimately leading to improved clinical outcomes.

In this Research Topic, we aim to deepen the understanding of the underlying mechanisms of glioma chemoresistance and to explore potential intervention strategies. Our goal is to advance the development of more effective therapies for glioma. We invite manuscripts that provide novel insights and significant advancements in overcoming chemoresistance in glioma. We especially invite research that addresses, but is not restricted to, the following topics.

Please note: Studies exploring pediatric glioma or focusing on metastatic disease to the brain do not fall within the scope of this Research Topic. Manuscripts consisting solely of bioinformatics, computational analysis, or predictions of public databases which are not accompanied by validation (independent clinical or patient cohort, or biological validation in vitro or in vivo, which are not based on public databases) are not suitable for publication in this journal.
