The central and peripheral airways, encompassing the trachea, mainstem, and peripheral bronchi, are pivotal for the conduction of gases during breathing and the drainage of tracheal and bronchial secretions. Disease manifestations such as neoplastic or benign tracheal stricture, malacia, or functional collapse, critically impair the airway's conductive capacity, leading to an increased work of breathing. Despite their significance, these conditions are often misdiagnosed as asthma or chronic obstructive pulmonary disease (COPD). Peripheral airway pathologies, including bronchopleural and alveolar-pleural fistulas, further complicate clinical management and present challenges that typically demand a multidisciplinary approach. Additionally, systemic inflammatory or autoimmune diseases may also compromise the tracheobronchial function, thus necessitating a deeper understanding and advanced treatment approaches.

This Research Topic aims to enhance the understanding and treatment of underdiagnosed central and peripheral airway diseases. By focusing on both systemic and endoscopic therapies, the goal is to highlight advancements in both diagnostic approaches and therapeutic interventions. It aims to address the complex interplay between these conditions and to provide insights into improved management strategies that may ease the burden of these diseases.
