Chronic ocular diseases such as age-related macular degeneration, diabetic retinopathy, glaucoma, and dry eye disease are among the leading causes of visual impairment and blindness around the globe. With the combined effects of an aging population and the rising prevalence of chronic metabolic disorders, the incidence of these diseases is increasing, thereby exacerbating their impact on healthcare systems and socioeconomic structures. These conditions, often marked by their gradual progression, culminate in irreversible vision loss significantly affecting patients' quality of life. Despite recent advancements in understanding disease mechanisms, including inflammation, neurodegeneration, and vascular dysfunction, significant gaps remain. Imaging and AI-assisted diagnostics are rapidly evolving and offer promising avenues for early intervention and precise disease management.

This Research Topic aims to create a nexus between basic research, clinical applications, and international perspectives to transform the management paradigms of chronic ocular diseases. The objective is to synthesize recent findings on pathogenic mechanisms to foster clinical translations of innovative imaging and diagnostic tools. Bridging disciplines, this effort encourages collaboration among various fields and professionals in order to develop effective strategies for prevention and management. Emphasis will also be placed on generating a global viewpoint regarding future research trajectories and comprehensive strategies in chronic ocular disease prevention.

To uncover further insights into the complexities of chronic ocular diseases, we invite contributions addressing, but not limited to, the following themes:
o Pathogenic mechanisms: roles of inflammation, immune dysregulation, oxidative stress, mitochondrial dysfunction, and neurodegeneration.
o Molecular and cellular interactions: microglial activation, macrophage polarization, and vascular–neural–immune crosstalk in disease progression.
o Biomarker discovery: identification of novel molecular, genetic, or imaging biomarkers for early detection and therapeutic monitoring.
o Diagnostic innovations: advances in multimodal imaging and quantitative imaging biomarkers.
o Artificial intelligence applications: application of machine learning and deep learning for personalized disease management.
o Therapeutic strategies: exploration of pharmacological, surgical, or regenerative approaches for chronic ocular disease management.
o Global perspectives: international studies and evaluations to understand global trends and disparities, aiming to enhance diagnostic access and health equity.
