Smoldering or low-grade inflammation plays a pivotal role in the context of both physiological and pathological conditions. For example, aging is accompanied by a physiological decline in immune competence, termed immunosenescence, which is characterized by inflammaging. Moreover, smoldering inflammation precedes and/or is associated with a variety of cardiometabolic disorders. Low-grade inflammation is associated with obesity and its complications such as diabetes and cardiovascular diseases. Immune cells (e.g., macrophages, mast cells, eosinophils), are strategically localized in white adipose tissue, an important source of pro-inflammatory cytokines in physiological and pathological conditions. Chronic low-grade inflammation also participates in the initiation and progression of several disorders of the immune system such as rheumatoid arthritis, psoriatic arthritis and allergic diseases. Reactive oxygen species (ROS) and reactive nitrogen species (RNS) are involved in physiological and pathological conditions. An imbalance between the production of ROS and the capacity of antioxidant defense mechanisms is implicated in asthma and several cardiovascular disorders. Angiogenesis plays a fundamental role in embryonic and post-natal development, reproduction and wound healing. Lymphangiogenesis plays a role in the resolution of inflammation in cardiovascular and allergic disorders. Inflammatory angiogenesis and lymphangiogenesis participate in the initiation and/or resolution of several pathological conditions. Smoldering inflammation can also be caused by non-resolving inflammation and is associated with several chronic diseases. The elucidation of the mechanisms that promote the resolution of inflammation is of paramount interest in physiology and pathology, and will reveal a pathway for pharmacological intervention and therapeutic development.
