One of the options available is cataract surgery with the implantation of more sophisticated intraocular lens (IOLs) that provide a greater depth of focus than the standard monofocal IOLs; the emphasis being to improve intermediate and/ or near vision together with distance vision leading to better all-round functional vision for patients with less dependence on spectacles.
Several types of specialised IOLs have been implanted over the years, including multifocal, enhanced depth of field (EDOF) and more recently, enhanced monofocal and trifocal IOLs. Over the years, the general trend amongst surgeons has been away from multifocal IOLs to EDOF and enhanced monofocals since multifocal IOLs are associated with more dysphotopsias and reduced contrast, which can be very troublesome for some patients. The more recent IOL solutions for presbyopia may not offer true spectacle independence for near reading vision, but they are a popular choice since they allow functional intermediate and distance vision with a much lower incidence of dysphotopsias.
Therefore, nowadays, there is a much greater choice of presbyopia correcting IOLs. A careful discussion of patients’ requirements and thorough clinical examination are mandatory before choosing the best options for individual patients. With this approach, patients can achieve very satisfying functional vision with less dependence on spectacles, with safer options available for the surgeon in terms of providing good quality dysphotopsia-free vision. Despite a careful patient-orientated approach, some people can be dissatisfied with the visual outcome, so the surgeon must allow time for careful and clear explanation of any problems experience by the patient and the possible solutions available.
This Research Topic welcomes submissions focusing on, but not limited to the optics of presbyopia and its correction, the design, and optics of the various IOL options available and how they differ, and careful patient selection and education to achieve the best possible results in cases of presbyopia.
