Prostate cancer is the most commonly diagnosed cancer and the second leading cause of cancer-associated mortalities in men in the United States. The first line of treatment for men with advanced prostate cancer is androgen ablation therapy. While initial responses are observed, unfortunately, the disease commonly recurs in its lethal metastatic form referred to as castration-resistant prostate cancer (CRPC). Although adenocarcinoma is the predominant histological subtype of CRPC, heavily treated tumors, particularly those treated with secondary hormone therapies, frequently acquire a neuroendocrine phenotype (neuroendocrine prostate cancer, NEPC), which currently accounts for 15-20% of CRPC and is commonly characterized by resistance to androgen deprivation therapies and an aggressive clinical course. Current therapies for CRPC including second-generation anti-androgens, chemotherapeutic agents such as taxanes, targeted therapies, and immunotherapy improve the median overall survival by only a few months.
Currently, there are two major clinical challenges in prostate cancer: distinguishing the patients who may benefit from treatments from patients who should not be treated but rather managed with active surveillance, and developing new and more effective therapies for metastatic prostate cancer.
Therefore, the goal of this Research Topic is to provide the most recent advances in prostate cancer early detection, model systems utilized to study the disease, molecular and cellular mechanisms underlying prostate cancer initiation, progression and neuroendocrine transdifferentiation, as well as emerging therapeutic targets and strategies for metastatic disease, and therapeutic approaches for prostate cancer.
