Spermatic granuloma of the epididymis-Ultrasography and MRI features:a case report and literature review

Sperm granuloma (SG) is a rare tumor-like lesion originating from the epididymis.Globally, the imaging findings of SG are poorly understood. This case report systematically recorded a SG confirmed by postoperative pathology, in a 75-year-old male. The patient reported a painless nodular mass in the scrotal area, identified through physical examination. Ultrasonography (US) revealed a well-circumscribed hypoechoic nodule within the epididymis, demonstrating peripheral vascularity on color Doppler. Magnetic resonance imaging (MRI) confirmed the presence of a T2-hyperintense lesion without diffusion restriction. This case report underscores the importance of differentiating spermatic granuloma from other scrotal pathologies with US and MRI, facilitating appropriate clinical management without unnecessary invasive procedures. A literature review is included, discussing the epidemiology, pathophysiology, and current diagnostic and management strategies for spermatic granuloma.
