Trend Analysis and Comparison of Anxiety Disorder Burden Among Women of Reproductive Age in China and Worldwide from 1990 to 2021

Objective: This study aimed to describe the changes in the age-specific burden of anxiety disorders among women of reproductive age in China from 1990 to 2021, including incidence, prevalence, and disability-adjusted life years (DALYs), and to compare these with the global burden of the disease. Methods: Using publicly available data from the Global Burden of Disease (GBD) database from 1990 to 2021, we analyzed the characteristics of the burden among women of reproductive age in China and globally, including changes in incidence, prevalence, and DALYs. Joinpoint regression was used to calculate the average annual percentage change (AAPC) and 95% uncertainty interval (UI) to reflect the trends in the burden of anxiety disorders among women of reproductive age. A comprehensive comparison was made across multiple dimensions, including age, gender, and time, to analyze the differences in the burden of the disease between China and other regions of the world. Results: From 1990 to 2021, China's age-standardized incidence rate (ASIR) for anxiety disorders climbed from 634.429 to 652.534 per 100,000, whereas the global ASIR surged from 649.317 to 784.532 per 100,000. China's age-standardized prevalence rate (ASPR) edged up from 4255.554 to 4380.91 per 100,000, contrasting with the global ASPR's escalation to 5535.608 per 100,000 (from 4674.187). China's age-standardized DALY rate (ASDR)rose from 508.114 to 525.577 per 100,000, while the global ASDR soared from 550.701 to 652.166 per 100,000. The AAPCs for ASIR, ASPR, and ASDR in China were -0.2582, -0.4587, and -0.4169, respectively, while the AAPCs for the global burden of the disease were 0.1875, 0.1009, and 0.1144, respectively. Age and gender both influenced the disease burden, and with economic development, the peak age of onset shifted later; the incidence, prevalence, and DALYs of anxiety disorders were higher in women across all age groups than in men. Conclusion: From 1990 to 2019, the incidence, prevalence, and DALYs of anxiety disorders among women of reproductive age in China showed a general decline, but a rapid increase occurred after 2019.
