Meta-analysis of correlation between duration of second stage of labor and recent postpartum pelvic floor injury in primiparous

Objective: To explore the correlation between the duration of the second stage of labor and recent postpartum pelvic floor injury in primiparous women. Methods: Randomized controlled trials (RCTs) were systematically evaluated by comprehensive literature review and inclusion criteria. To evaluate the correlation between the duration of the second stage of labor and recent postpartum pelvic floor dysfunction in primiparous women. The quality of the study was evaluated using the Cochrane risk assessment tool for bias and the RevMan 5.3 software was used for Meta-analysis. Primary outcome measures included POP, UI, FI, postpartum sexual dysfunction, pelvic floor pain, and urinary retention. Results: A total of 14 articles were included, and there were 3059 researchers, 1506 cases in the extension group and 1553 cases in the general group. All of them were randomized controlled trials. There was no significant difference between POP in the extension group and the normal group (OR=1.25, 95% 95%CI [0.99-1.59]], P = 0.06). UI was more severe in the extension group (OR=1.48, 95% 95%CI [1.07-2.05]], P = 0.02). There was no significant difference in FI between the extension group and the normal group (OR=1.28, 95% 95%CI [0.98-1.68]], P = 0.07). The postpartum sexual dysfunction in the extension group was not significantly worse than that in the control group (OR=1.26, 95% 95%CI [0.72-2.21]], P = 0.42). The condition of pelvic floor pain was stronger in the extension group (OR=1.24, 95% 95%CI [1.03-1.50]], P = 0.03). The condition of urinary retention was more severe in the extension group (OR=2.40, 95% 95%CI [1.12-5.15]], P=0.03). Conclusion: The prolongation of the second stage of labor in primiparous parturients is significantly correlated with recent postpartum pelvic floor dysfunction such as urinary incontinence, pelvic floor pain and urinary retention. In pelvic organ prolapse and fecal incontinence, although there is no significant difference, but there is a certain trend; Postpartum sexual dysfunction did not show a significant correlation with prolonged labor.
