Effect of prior diverse surgical histories on the efficacy and safety of one-stage retrograde intrarenal surgery in patients with renal stone: a retrospective study

Abstract

Purpose: To explore the impact of prior percutaneous nephrolithotomy (PCNL) or retrograde intrarenal surgery (RIRS) on the efficacy and safety of one-stage RIRS for renal stone patients. Methods: A retrospective analysis was performed on 115 renal stone patients who underwent one-stage RIRS between January 2022 and June 2025. They were divided into three groups: Group A (prior PCNL, 28 cases), Group B (prior RIRS, 37 cases), and Group C (no urinary stone surgery history, 50 cases). Operative time, postoperative hospital stay, postoperative serum creatinine elevation, complication rate, and stone-free rate (SFR) at 1 and 4 weeks postoperatively were compared among the three groups. Results: All patients completed the surgery successfully. Operative time in Group A was (61.5 ± 8.9) min, significantly longer than that in Group B [(53.4 ± 6.3) min] and Group C [(51.0 ± 6.7) min] (P<0.01). No significant differences were found in postoperative hospital stay (Group A: 2.2 ± 0.8 d; Group B: 2.2 ± 0.7 d; Group C: 2.1 ± 0.3 d), postoperative serum creatinine elevation (Group A: 12.23 ± 8.68 μmol/L; Group B: 13.34 ± 9.11 μmol/L; Group C: 12.16 ± 8.38 μmol/L), or complication rate (Group A: 7.1%; Group B: 8.1%; Group C: 8.0%) (all P>0.05). At 1 week postoperatively, SFR was 64.2% (Group A), 62.2% (Group B), and 66.0% (Group C); at 4 weeks, it was 85.7% (Group A), 89.2% (Group B), and 90.0% (Group C), with no significant intergroup differences (all P>0.05). Conclusion: Prior PCNL may prolong the operative time of one-stage RIRS but does not significantly affect SFR or complication rate. Prior RIRS has no significant impact on operative time, SFR, or postoperative complications. One-stage RIRS is an effective and safe treatment option for renal stone patients with prior PCNL or RIRS.
