Chronic Disease Prevention in Primary Care

Chronic disease prevention represents a pivotal focus within the realm of primary care, given the rising global burden of conditions such as cardiovascular diseases, obesity, chronic kidney disease, and diabetes. Despite the well-established impact of these diseases on morbidity and mortality, there remain significant challenges in integrating effective prevention strategies in everyday clinical practice. Current questions revolve around the best approaches for risk assessment, early intervention, and patient engagement within various population groups. Debates also persist regarding the optimal use of resources, the implementation of evidence-based guidelines, and the role of technology in supporting preventive care.

Recent studies attest to the effectiveness of tailored lifestyle interventions, routine screening programs, and collaborative care models in mitigating chronic disease development. Nevertheless, the translation of these findings into routine primary care remains inconsistent. Digital health tools, multidisciplinary teams, and patient self-management programs offer promising avenues; yet, gaps endure in addressing social determinants of health, reducing healthcare disparities, and ensuring sustainable behavior change. More attention is needed to generate scalable, equitable interventions that fit the unique constraints and opportunities of primary care.

This Research Topic aims to highlight innovative strategies and best practices for the prevention of chronic diseases within primary care settings. The objective is to explore new models of care, examine barriers and facilitators to implementation, evaluate patient and provider perspectives, and identify effective policy and system-level interventions. Through a multidisciplinary lens, this collection seeks to synthesize knowledge and contribute to a more robust evidence base guiding prevention efforts in primary care.

To gather further insights into approaches, opportunities, and limitations in chronic disease prevention through primary care, we welcome articles addressing, but not limited to, the following themes:
