TyG Index and CBC-Derived Inflammatory Indicators Individual and Mixed Effects on All-Cause and Cardiovascular Disease Deaths in Patients with CHD

Background and Aims: Most epidemiological studies focus on single indicators for cardiovascular disease (CVD) risk, yet individual markers often inadequately reflect patient health. A comprehensive assessment using multiple indicators is crucial. The Triglyceride-Glucose (TyG) index and Complete Blood Count (CBC)-derived inflammatory markers are gaining attention, but research on their individual and combined impact on all-cause and cardiovascular mortality risk in coronary heart disease (CHD) patients is limited. This study aimed to develop a more accurate clinical risk assessment tool for coronary artery disease (CAD) management. Methods and Results: CHD patients aged ≥20 years were identified from five NHANES cycles (2009-2018). Weighted logistic regression and restricted cubic spline (RCS) analyses assessed the TyG index and CBC-derived inflammatory markers (SII, SIRI, MLR, NLR, PLR) for association with all-cause and cardiovascular mortality. A Weighted Quantile Sum (WQS) model estimated their cumulative exposure effect. In unadjusted models, ln(SII), ln(SIRI), ln(MLR), ln(NLR), and ln(PLR) were positively associated with all-cause mortality risk (all p<0.05, ORs ranging 1.8-2.57). After full confounder adjustment, these associations remained significant. RCS analysis showed nonlinear dose-response relationships between MLR, NLR, PLR, SII, SIRI, and all-cause mortality (P-nonlinear <0.05). The WQS model indicated that combined exposure to the TyG index and CBC inflammatory markers significantly increased risks for all-cause mortality (OR=1.68, 95% CI=1.22-2.30, P=0.009) and cardiovascular death (OR=2.2, 95% CI=1.42-3.42, P=0.0004) in CAD patients, with NPR and SII contributing most substantially to the overall risk. Conclusion: The TyG index and CBC-derived inflammatory indices are significant risk factors for all-cause and cardiovascular mortality in CHD patients, with NPR and SII representing the largest contributors within these factors.
