Background

Heart failure remains a leading cause of morbidity and mortality worldwide, with limited options for true myocardial regeneration. MicroRNA (miRNA) interference has emerged as a promising therapeutic approach due to miRNAs’ pivotal role in regulating gene expression, particularly in cardiomyocyte proliferation, survival, and differentiation. Despite encouraging preclinical findings, the translation of miRNA-based strategies from bench to bedside faces considerable challenges.

This Research Topic aims to provide an in-depth examination of the current advances, limitations, and open questions in miRNA interference as applied to heart failure. We strongly encourage the submission of original research articles, reviews, perspectives, and meta-analyses that investigate the molecular mechanisms by which miRNAs modulate cardiac biology, the design and optimization of miRNA mimics or inhibitors, and preclinical studies addressing safety and delivery methods. Submissions focusing on spatio-temporal regulation of miRNA activity, mitigation of off-target effects, and integration with current pharmacotherapies are especially welcome. We also invite explorations into the integration of miRNA interventions with genomic and proteomic data for precision medicine applications.

The overall aim is to foster a comprehensive dialogue among scientists and clinicians about the therapeutic promise of miRNA interference in heart failure management. Emphasis will be placed on dissecting the obstacles related to delivery systems, immunogenicity, toxicity, and efficacy in human models. Additionally, the Research Topic is positioned to spotlight ongoing debates and future directions, thereby providing readers with a roadmap for overcoming translational barriers and leveraging miRNA-based treatments to enhance cardiac repair, ultimately paving the way for more individualized and effective cardiovascular therapies.
