Primary adrenal melanoma is an exceedingly rare diagnosis. We present a new case of primary left adrenal melanoma with rapid and extensive distant metastasis. The patient underwent a left adrenal tumor biopsy and the pathology result showed an adrenal malignant melanoma. Immunohistochemical staining showed positive antibody-specific cytoplasmic reaction to Melan A, HMB45 and SOX-10. We reviewed the relevant cases that have been reported in the published English literature and discussed the diagnostic criteria, differential diagnosis and possible management.
