Objective: To compare the effectiveness and safety of External Beam Radiotherapy (EBRT) versus Portal Vein Stent Implantation (PVSI) when combined with local interventional therapy and TKI plus ICIs in patients with hepatocellular carcinoma (HCC) and portal vein tumor thrombus (PVTT), providing real-world evidence for clinical decision-making. Methods: This retrospective study compared the effectiveness of EBRT versus PVSI when combined with local intervention and TKI plus ICIs. We assessed objective response rate (ORR), disease control rate (DCR), overall survival (OS), progression-free survival (PFS), and safety outcomes, including complications and liver injury. Univariate and subgroup analyses identified prognostic factors to support personalized treatment decisions.
