The landscape of gynecological oncology is experiencing a paradigm shift, with increasing emphasis on fertility preservation strategies for women diagnosed with endometrial cancer. This concern is particularly pronounced among younger patients who wish to retain their reproductive potential—a challenge compounded by the recent revolution in our molecular understanding of endometrial cancer. The integration of molecular classification into clinical practice is fundamentally changing treatment paradigms, allowing for more nuanced, individualized approaches that can reconcile oncological safety with fertility preservation.
Current research is moving beyond generic eligibility criteria for conservative management, instead focusing on how molecular profiles influence the appropriateness and efficacy of fertility-sparing treatments. Studies now seek to elucidate the impacts of such therapies on reproductive organs, while also evaluating innovative interventions that may improve the prospects of conception post-recovery. Despite these advances, significant gaps remain in our knowledge regarding the optimal balance between effective cancer management and preservation of fertility. The advent of molecular classification holds the potential to bridge these gaps, offering a framework for tailored treatment decisions and outcome prediction.
This Research Topic aims to advance our understanding of fertility preservation in women with endometrial cancer, with a specific focus on the implications of molecular classification. Key objectives include exploring the relationship between molecular subtypes and both established and emerging fertility-preserving strategies, assessing the effectiveness and safety of these interventions, and reporting on patient-centered outcomes including reproductive and obstetric results.
