Anti-Amyloid Immunotherapy in Alzheimerʼs Disease: Two Years of Clinical Experience and Emerging Real-World Evidence

Since the landmark approvals of lecanemab and donanemab, anti-amyloid immunotherapy has marked a paradigm shift in the pharmacological treatment of Alzheimerʼs disease. Beyond controlled trials, clinicians now face questions of implementation, monitoring (including ARIA), biomarker validation, treatment sequencing, and equitable access. Regional differences in health policy, reimbursement, and infrastructure have produced variable adoption rates, creating an urgent need for shared global and pharmacological insights.

The goal of this Research Topic is to synthesize global experience after two years of clinical use of anti-amyloid monoclonal antibodies for Alzheimerʼs disease, with a focus on pharmacology, psychopharmacology and neuropsychiatric symptoms of Alzheimer’s disease. By integrating trial data, post-approval findings, and real-world evidence (RWD), this collection will define how disease-modifying therapy is reshaping diagnosis, treatment algorithms, and health systems. It will also highlight emerging ethical and policy challenges as these therapies transition into daily practice worldwide.

This Research Topic will cover: (1) profiles of approved and emerging antibodies; (2) biomarker-guided diagnosis and monitoring; (3) real-world data from the United States, Europe, Asia, and Japan; (4) sequential and combination therapy including anti-tau agents; (5) address the understanding and treatment of neuropsychiatric symptoms in Alzheimer’s disease, including their neurobiological underpinnings, pharmacological management and (6) ethical, policy, and access challenges. We invite Reviews, Mini-Reviews, Original Research, and Policy Papers addressing the clinical, scientific, and public health aspects of anti-amyloid immunotherapy.
