Human Epidermal Growth Factor Receptor 2 Expression in Gynecologic Serous Carcinomas and Preliminary Analysis of Its Characteristics in Patients Post-neoadjuvant chemotherapy

Objectives: To investigate HER2 status in uterine (USC) and ovarian high-grade serous carcinoma (HGSC), compare ASCO/CAP and ISGyP interpretation criteria, and analyze prognostic significance. Methods: We retrospectively analyzed 51 USC and 165 ovarian HGSC cases. HER2 was assessed by IHC using ASCO/CAP (2023) and ISGyP criteria, with FISH validation. Survival analysis was performed. Results: HER2 amplification rates were 29.4% in USC, 4.9% in primary ovarian HGSC, and 7.0% in post-neoadjuvant HGSC. HER2 3+ rates were similar between criteria, but ISGyP identified more HER2-low cases. HER2 positivity correlated with worse prognosis in ovarian HGSC. Conclusion: HER2-positive and HER2-low cases exist in serous carcinomas. ISGyP criteria are more sensitive for HER2-low. HER2 status provides prognostic and therapeutic implications.
